WAIVER & RELEASE OF LIABILITY – REAL BOXING CLUB LLC
1. Legal Guardian / Adult Representation
I expressly warrant and represent that:

If the member is a minor, I am the Parent or Legal Guardian of {name} and have full legal authority to agree to the terms contained in this Waiver and Release of Liability on their behalf;

If the member is a legal adult, I am signing this Waiver and Release of Liability on my own behalf.
2. General Activities
I understand that the member (my ward or myself) may participate in all activities at REAL BOXING CLUB LLC, including but not limited to:

Physical exercises and equipment use

Sparring and other contact activities

Participation in competitions

Classes, training, and instruction by personal trainers
I understand that sparring is an optional but common training activity in boxing and involves controlled physical contact between participants, including punches delivered while wearing protective equipment such as gloves and mouthguards.
I acknowledge that sparring carries a higher risk of injury than non-contact training, including but not limited to concussion, head injury, facial injury, cuts, bruises, or other bodily harm.
By signing this agreement, I expressly consent to the participation of the member (my ward or myself) in sparring activities conducted under the supervision of REAL BOXING CLUB LLC coaches. I understand that participation in sparring is voluntary and may be declined at any time.

3. Health Representation & Assumption of Risk
The Club encourages all participants to undergo a medical examination by a doctor before using equipment or participating in any exercise activity.
I understand that boxing is a combat sport involving punches, physical contact, and strenuous physical activity. I acknowledge that participation in boxing training, sparring, and related activities may involve risks of serious injury, including but not limited to concussion, head injury, broken bones, sprains, cuts, bruises, or other bodily harm.
The member (my ward or myself) represents that they are in good health and have no disability, impairment, or illness that prevents safe participation, or that poses a risk to the health of others.
The member agrees not to use any REAL BOXING CLUB facilities with open cuts, abrasions, ulcers, infections, or infectious diseases, in accordance with state and local health requirements.
The administration reserves the right to prohibit or restrict access to the Club for members with such conditions.
The member (my ward or myself) assumes all risks and responsibility for injuries sustained as a result of participation in activities, use of facilities, and instruction by trainers at REAL BOXING CLUB LLC.
In the event of an injury or medical emergency, I authorize REAL BOXING CLUB LLC and its staff to obtain emergency medical treatment for the participant if deemed necessary. I understand that I am responsible for any medical expenses incurred.

4. Personal Property
REAL BOXING CLUB urges participants not to bring valuable personal property.
The Club, its officers, employees, contractors, and agents shall not be liable for loss, theft, or damage to personal property, including lockers.
The member (my ward or myself) waives all claims against the Club for such loss.
The Club reserves the right to remove, dispose of, or sell items left overnight in day-use lockers.

5. Photo & Video Release
I hereby give permission to REAL BOXING CLUB LLC to use any photos or videos of the member (my ward or myself) for promotional purposes, in any media now known or hereafter developed.

If signing on behalf of a minor, I am the Parent or Legal Guardian and consent to the use of images of my ward.

If signing for myself as a legal adult, I consent to the use of my own images.
I understand that this consent is voluntary and that I will not receive any compensation for the use of these images.

6. Property Damage
Any participant (my ward or myself) whose negligence causes damage to equipment, fixtures, or facilities will be responsible for repair or replacement.

7. Violations of Rules
The administration reserves the right to terminate membership if any rules are violated.
Termination of membership for rule violations does not release the member from obligations to pay any dues accrued up to the date of termination.

8. Waiver of Claims
The Member (my ward or myself) hereby releases, waives, and discharges REAL BOXING CLUB LLC, its owners, officers, employees, coaches, contractors, and agents from any and all claims, demands, injuries, damages, actions, or causes of action arising out of or related to participation in any activities at the Club, including but not limited to boxing training, sparring, competitions, classes, instruction, or the use of equipment and facilities.
This release applies to any and all injuries or damages, including those caused by the active or passive negligence of REAL BOXING CLUB LLC, its owners, employees, coaches, contractors, or agents, to the fullest extent permitted by the laws of the State of Florida.

9. Severability
If any provision of this Waiver and Release of Liability is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

10. Governing Law
This Waiver and Release of Liability shall be governed by and interpreted in accordance with the laws of the State of Florida. Any legal action arising from this agreement shall be brought exclusively in the courts located in Hillsborough County, Florida.

I acknowledge that I have carefully read this Waiver & Release of Liability, understand its contents, and agree to all terms.
