General Activities:
I expressly warrant and represent that I am the legal guardian of {name} on whose behalf I am completing this registration, and have full legal authority to agree to the terms contained in this Waiver and Release of Liability.
 

In this document, the member(s) of REAL BOXING CLUB LLC, on whose behalf I am going through this registration, is referred to as "my ward".  I understand that I am giving my ward permission to participate in all activities, including all physical active games and exercises, sparring, participation in competitions and other activities.

Electronics/Personal Property:
REAL BOXING CLUB urges members not to bring valuable personal property into the gym. REAL BOXING CLUB, its officers, employees, independent contractors and agents shall not be liable for the loss, theft of or damage to the Member's personal property located anywhere in REAL BOXING CLUB, including all lockers, and locker rooms. Member and Buyer further agree to waive such claims against REAL BOXING CLUB, its officers, employees, independent contractors and agents. REAL BOXING CLUB reserves the right to remove, dispose of and/or sell all articles left overnight in the day use lockers without prior notice. All Lockers and cubbies are for day use only.

Publicity release:
REAL BOXING CLUB LLC may use any photos or video of me/my ward for promotion of REAL BOXING CLUB LLC.

Assumption of Risk and Indemnity:
REAL BOXING CLUB shall not be liable for any claims, demands, injuries, damages, actions or causes of action whatsoever, to members or property arising out of or connected with the use of any facilities of REAL BOXING CLUB of the premises where the same are located, and member does hereby expressly forever release and discharge the said REAL BOXING CLUB, its officers, employees, independent contractors or agents from all such claims, demands, injuries, actions, or caused of action, and from all acts of active or passive negligence of the part of such company, corporation, club, its servants, agents or employees. The Member and the Purchaser assume all risks and responsibility for any injuries sustained by the Member (and/or child/ward) as a result of his/her physical exercise, use of facilities and equipment, participation in classes/competitions and/or instruction by a personal trainer and any other activities in and outside the Gym.

I acknowledge that I have carefully read the above Waiver and Release of Liability, understand its contents, and I recognize that by signing this legally binding agreement on behalf of myself and my ward and other parents and guardians of my ward, we are freely giving up substantial rights and assuming all risk of injury, death and property damage. I understand this document and agree to all content described.
